Dear Providers: To meet documentation requirements for consultation requests, Orthopaedic Surgery Associates of Marquette, P.C. has implemented a "Consultation Request Form". Please use this form when requesting consultations. We are no longer able to accept verbal consultation requests.
Consultation defined: Requesting provider is seeking advice or opinion on a specific problem and is not transferring all or a portion of the patient's care.
